Changing your last name in Florida
He stole your heart and you want to steal his last name? You have two options to do it the first one is to do it yourself and the other to pay a name changing company to save you hours. If you decide to do it yourself…
Here are the steps you need to follow to make that happen;
The most important step to start this change is to get certified copies of your marriage certificate from the local clerk or county office your marriage license was filed in. After you have this documents on your hand you will need to change your name on your:
-Social Security Administration (SSA)
-Florida Department of Highway Safety and Motor Vehicles
Social Security Administration (SSA)
Visit your local Social Security Administration office to change your name so you can get a new Social Security card that reflect your new name. You may change your name either in person or by email. You cannot change your name online. In order to that you need the following documents:
-A completed Application for Social Security Card (Form SS-5)
-Your certified name-change document
-Proof of your identity; U.S. driver’s license, U.S. passport or State-issued ID Card
-Proof of you U.S. citizenship; U.S. birth certificate, Certificate of citizenship or Certificate of naturalization
It is important to note that all documents must be original or certified copies. Mail the above documents or bring them to your local Social Security Administration Office.
Florida Department of Highway Safety and Motor Vehicles
Change your name at the Department of Motor Vehicles after you receive your new Social Security Card to get a new Driver’s License. You can only change your name in person at any local Florida Division of Driver Licenses office. You will need the following documents:
-Florida Driver license or ID card
– Proof of you U.S. citizenship; U.S. birth certificate, Certificate of citizenship or Certificate of naturalization, Valid U.S passport
-Proof of your current name; Marriage Certificate
-Proof of Social Security Number; utility bill, W-2 form, Ineligibility letter from the SSA and 1 document from the following list GatherGoGet list.
-Proof of Florida residency (2 documents); Utility Bill, bank statement, school record or transcript
There is a fee to be paid to change your last name, the fee is $25. Also, all documents must be originals or certified copies
The following step is not mandatory but you might take into consideration
FL Registration and Title Name Change
If your name has been changed on you FL driver license, you do not need to update your registration record separately. However, if you want to change the name on your vehicle title and/or registration card you must follow these steps:
-Complete your title to transfer it to the correct name
-Submit signed title in person at your local county tax collector’s office
-Fees: $75.25 corrected electronic title fee and $2.50 if a printed copy is desired
After all this important steps have been follow you must change your names in the following places:
-At work, this change will be made to your W-2, 2-4 and deductions if needed
-Change your name and address with the United State Postal Service
-Change your name on your Bank accounts to keep your identity intact
-Change name on your passport
-Notify other places you need to change your name
